By law, the Mississippi Farm Bureau Health Plan (the “Farm Bureau Plan”) is required to protect the privacy of your protected health information. We also must give you this notice to tell you how we may use and release (“disclose”) your protected health information held by us.   The Farm Bureau Plan is sponsored by Mississippi Farm Bureau Federation and provided through its wholly owned subsidiary, Mississippi Farm Bureau Health Plan, Inc.  The Farm Bureau Plan has selected Arkansas Blue Cross and Blue Shield to administer the Farm Bureau Plan.  Arkansas Blue Cross is a business name of USAble Mutual Insurance Company.  As a part of the administration of the Plan, Arkansas Blue Cross is responsible for providing a Notice of Privacy Practices to Plan members.  The Arkansas Blue Cross Notice of Privacy Practices can be found here: https://www.arkansasbluecross.com/company/privacy-notice
